By scheduling an appointment with Woden Dental Centre (“the Practice”), you agree to be bound by these Terms and Conditions. Please read them carefully before proceeding with any dental services.
2.1 Scheduling
- Appointments must be scheduled in advance
- Valid contact information must be provided
- Confirmation of appointments may be required via phone or email
2.2 Cancellations
- 24 hours notice is required for cancellations
- Late cancellations or no-shows may incur a fee
- Repeated no-shows may result in requiring pre-payment for future appointments
3.1 Fees
- Payment is required at the time of service
- Private health insurance claims can be processed on-site through HICAPS
- Cost estimates will be provided before beginning treatment
3.2 Insurance
- Patients are responsible for understanding their insurance coverage
- Gap payments are the patient’s responsibility
- The Practice will assist with insurance claims but cannot guarantee coverage
4.1 Consent
- Written consent is required for all major procedures
- Parents/guardians must provide consent for patients under 18
- Patients have the right to refuse treatment at any time
4.2 Treatment Plans
- Comprehensive treatment plans will be provided
- Multiple treatment options will be presented where applicable
- Treatment plans may be modified based on clinical findings
5.1 Information Disclosure
- Accurate medical history must be provided
- Changes in medical conditions must be reported
- Current medications must be disclosed
- Contact information must be kept up to date
5.2 Behaviour
- Respectful behaviour towards staff is required
- Compliance with safety protocols is mandatory
- Following post-treatment instructions is essential
6.1 Standards of Care
- All treatment will meet Australian Dental Association standards
- Licensed professionals will perform all procedures
- Sterilization and safety protocols follow current guidelines
6.2 Privacy
- Patient information will be handled in accordance with the Privacy Act 1988
- Records will be maintained securely
- Information will only be shared with authorized parties
- After-hours emergency contact information will be provided
- Emergency appointments will be prioritized when possible
- Additional fees may apply for after-hours emergency care
8.1 Complaints Process
- Complaints should be submitted in writing to the Practice Manager
- All complaints will be investigated thoroughly
- The response will be provided within 14 business days
8.2 Resolution
- Disputes will be handled fairly and promptly
- External mediation may be used if necessary
- Dental Board of Australia guidelines will be followed
- Terms and conditions may be updated periodically
- Notice of changes will be posted in the Practice
- Continued use of services constitutes acceptance of new terms
